Position Description

Job Title: Advanced Practice Clinician (NP/PA) – Senior Community Care

Location: Guilford County, NC Schedule: Monday–Friday | 8:00 AM – 5:00 PM | Full-time (40 hours/week) Contract Duration: 3–4 months (Temporary coverage for FMLA leave beginning 8/1) Start Date: ASAP (to allow for training prior to leave) Travel: Required between multiple senior living facilities within Guilford County Dress Code: Medical attire (no specific scrub color required) Interview Process: Virtual interview via Microsoft Teams

About The Role

Join our Senior Community Care (SCC) team, a longitudinal, integrated care delivery model designed to bring high-quality healthcare directly to facility-based patients. As an Advanced Practice Clinician (APC), you'll work closely with an interdisciplinary team to care for high-risk health plan members residing in senior living environments.

You'll provide patient-centered care while enjoying flexibility—no nights, weekends, or holidays required—making work-life balance a reality.

✅ Primary Responsibilities

  • Conduct in-depth, comprehensive assessments across clinical domains
  • Manage both acute and chronic medical and behavioral conditions
  • Accurately document diagnoses (including ICD-10) and clinical encounters
  • Facilitate advanced illness and end-of-life care discussions
  • Collaborate with interdisciplinary care teams
  • Educate and support patients and caregivers through counseling

Required Qualifications

  • Active, unrestricted NP or PA license in North Carolina (or ability to obtain)
  • For NPs: Master's or Doctorate in Nursing; certified in Family, Adult, Gerontology, or Adult-Gerontology Acute Care
  • For PAs: Graduate of an accredited PA program; NCCPA board certified
  • Active DEA licensure and prescriptive authority
  • Full COVID-19 vaccination in accordance with applicable laws and policies
  • Physical ability to lift up to 30 lbs, enter varied dwelling conditions, and complete assessments
  • Access to reliable transportation; willing to travel between facilities during shifts
  • Ability to obtain collaborative practice agreement if required by state

✨ Preferred Qualifications

  • 1+ years of clinical experience (senior living or long-term care preferred)
  • Experience with medically complex patients
  • Strong understanding of geriatrics, chronic illness, and advanced care planning
  • Proficiency in electronic and written medical documentation systems
