Position Description

Nurse Practitioner / Physician Assistant 

Provides highly personalized primary care and chronic disease management through an innovative care model across multiple settings. Care may be delivered in clinics, patient homes, nursing facilities, or other locations. Participates in population-based care of frail, elderly patients in collaboration with a multidisciplinary team that includes physicians, case managers, and social workers.

General Duties/Responsibilities: (May include but are not limited to)

  • Collaborates with primary care physician, Extensivist, and Case Manager to develop care plans for members.
  • Performs diagnostic and/or therapeutic procedures within his/her level of training and expertise, and as outlined in the practice agreement and written protocol with the supervising physician.
  • Orders, interprets, and evaluates diagnostic tests to identify and assess patients’ clinical problems and health care needs.
  • Discusses cases with physician when appropriate.
  • Prescribes medication or other forms of treatment as indicated.
  • Use of Electronic Medical Records required.

Supervisory Responsibilities:

  • None.

Minimum Requirements:

  • Minimum Experience:
  • 1 year related experience with clinical emphasis for Nurse Practitioner or Physician Assistant; or any combination of education and experience, which would provide an equivalent background.
  • Education/Licensure:
  • Master’s Degree in Nursing or Graduate of an accredited Physician's Assistant program required.
  • Current unrestricted RN license and NP license in applicable state; or PA license in applicable state and current NCCPA certification required.
  • Prescribing authority/DEA licensure in good standing as required in the state in which you are applying preferred.
  • Other:
  • Epic EMR experience is strongly preferred.
  • Must be flexible with traveling between centers, patient homes, or other facilities to meet clinical needs.
